Registration forms and payment are due to JEB Stuart Crew registrar, Mari Mullane, by November 16.

In case you missed picking up a registration packet at the General Interest meeting on October 28, crew forms are online. Click on the Forms/Links tab to get started. Only the Athletic Participation/Parental Consent/Physical Examination Form required by the VIRGINIA HIGH SCHOOL LEAGUE, INC. goes to the JEB Stuart Activities Office. Submit completed form to the Activities Office in person, or email phys ed form to kbberg@fcps.edu. Note that the phys ed form requires a doctor’s visit; please allow enough time to get through this step.

Make check/s payable to ”JEB Stuart Crew Boosters” and submit check (either full payment or the first installment) and forms to the JEB Stuart Crew Mail Box in the JEB Stuart High School Front Office.
